The medical form
Every diver declares their fitness to dive before getting in the water. The portal keeps that declaration on the client's record and shows, at a glance, whether it's complete or missing.
The Blue Trails questions
There's a baseline set of questions every dive center on Blue Trails asks — about heart and lung conditions, asthma, age, ear and sinus issues, fitness to exercise, and so on. These are the standard medical screening questions and are read-only: they're the same everywhere so a diver's answers travel with them.

Your own questions
On top of the baseline you can add your own questions, specific to your center (see Medical questionnaire in settings). Turn them on and your linked divers answer them in the app.
Completing a form
A diver's form can be filled in three ways:
- the diver fills it in the app, if they're linked,
- the diver fills it via a QR code you show them (see Public sign-up by QR),
- or you record it for them in the client editor.
The PDF
A completed medical form can be produced as a PDF for your records or to keep on file — and you can print a blank one from the Clients page for divers who'd rather fill it on paper.
