Medical questionnaire

Beyond the standard Blue Trails health questions, you can run your own questionnaire — extra questions specific to your center. Open Settings → Medical questionnaire.

Your own questionnaire

This is a separate questionnaire, not an addition to the Blue Trails one. Turn it on and your linked divers find it in the app; add, edit, reorder or switch individual questions on and off.

Use it for anything the baseline doesn't cover — a local waiver point, a center-specific health note, a consent question.

The Blue Trails questions

Below your own, you'll see the Blue Trails questions: the baseline every center on the platform asks. These are read-only — they're shared across Blue Trails so a diver's answers are consistent wherever they dive.

How divers answer

Divers complete the questionnaire in the app (if linked), through your QR sign-up, or you record it for them on their client record.
